These five forms transformed my classroom. They made following up with student absences or missing work and guiding students through my expectations a breeze.

Criteria for Success
I have a fellow student fill out a Absent Student Report for students who are absent. It is then stapled to any work the student missed and put in a file for students to pick up the next day. The best part is that it has helped to remind me to have a brief conversation with the student about the concept(s) they missed. Click the image for a FREE copy!
Students are asked to complete an Infraction Notice Report upon receiving their second warning in class for a particular behavior (i.e. talking out of turn). The form guides students through reflection on their behavior and the reasons behind it. Upon the third warning, a letter is sent home, and I meet one-on-one with the student to work together in finding a solution for their behavior. Click the image for a FREE copy!
When coming to class with incomplete work, students are asked to fill out this form. This form allows the student and I to see the reason behind their missing or incomplete work as well as to track the reasons for repeated occurrences. The best part is, it allows me to put a comment in the gradebook sharing with the student's parent/guardian why their work is missing or incomplete. Click the image for a FREE copy.
Students are asked to fill this out each time they turn in an assignment. This form guides students through my expectations regarding homework including completion and corrections of homework. This form is used to guide students through the test correction process. They can quickly select what mistake they made after reflection on their work and include a correct solution to the problems they missed.
